GLP-1 drugs— the class that includes well-known weight-loss and diabetes drugs — come up in almost every conversation about Medicare drug coverage these days. Here’s the honest, current answer for anyone comparing plans during this year’s Annual Enrollment Period (October 15 – December 7, 2026).
The Short Version
Medicare currently does not cover GLP-1 drugs
when they’re prescribed solely for weight loss. Federal law specifically excludes drugs used only for weight loss from Part D coverage. If a GLP-1 medication is prescribed for an approved medical use — such as managing type 2 diabetes, or for FDA-approved indications beyond weight loss like reducing cardiovascular risk — it may be covered under your Part D or Medicare Advantage drug plan, subject to that plan’s formulary and any prior authorization requirements.
What’s Happening in Washington Right Now
CMS has proposed a demonstration model that would allow Part D coverage of GLP-1 medications specifically for obesity and weight loss — something current law otherwise doesn’t allow. For the model to move forward for 2027, a large share of Part D plans need to formally apply to participate, and CMS has been evaluating whether that participation threshold will be met.
In plain terms: this is a real possibility on the table, but it is not yet guaranteed, and it would only apply if and when CMS confirms the model is moving forward. Nothing changes automatically — if it happens, it would affect which plans offer this coverage and under what conditions.
What This Means for You This AEP
If You’re Taking a GLP-1 for Diabetes or Another Approved Condition
Check your plan’s formulary carefully, since coverage, tier placement, and prior authorization rules vary by plan and can change year to year.
If You’re Hoping to Use a GLP-1 for Weight Loss Specifically
It is not currently covered by Medicare, and you should budget accordingly unless and until federal policy changes.
Either Way, This Is a Fast-Moving Area
Even if a plan doesn’t cover GLP-1s for weight loss today, that could shift for a future plan year, so it’s worth asking about specifically when comparing options.
